The 18.00 mm figure is the outside diameter of the guide — the size the bore in the cylinder head is machined to as standard — and says nothing about the valve stem running inside it. Exhaust guides sit in the hottest part of the head and receive the least oil, so they are usually the pair found worn when a head is stripped. Confirm the head bore has not already been opened out for a repair-stage guide before ordering this size.
